Preparing For Your Visit

Please Bring the Following to Your Appointment

  • Drivers License or ID Card
  • Current Health Insurance card(s)
  • Method of Payment (if you have copay)
  • Referrals and Medical records, unless faxed to us prior to your visit 
  • List or bottles of ALL medications you take
  • We do check in and paperwork on IPad in our office, so come 5-10 minutes before your New Patient appointment
  • We usually do two initial visits. First one is to do physical exam and order tests, and second one is to go over test results and determining best treatment strategies.

Copayments and Insurance Deductibles

Copayments and insurance deductibles represent the portion of our services that patients must pay out of pocket. Such payments are required by the patient’s agreement with the insurance payer, and our contracts with insurance payers require us to collect them. 

Review Our Guidelines

  • If you experience cold like symptoms please call us to reschedule your appointment.
  • Patients arriving more than 10 minutes after their scheduled appointment time will be marked as a no-show and must be rescheduled. 
  • Appointments must be cancelled at least 24 hours in advance to avoid no-show fee. Please call us to cancel or reschedule. It is okay to leave a voicemail or send a text to 727.290.1444 after hours - we will log the time of your voicemail/text.
  • Insurance information must be verified at each visit. If you have any changes to your insurance, notify us ASAP, so we can confirm your coverage. Denied claims will be the patient's responsibility.
  • We encourage patients to settle account balances quickly. If you are experiencing financial difficulty, talk to our billing team to discuss payment options. Balances over 90 days old will be transferred to collections, so don't wait to talk to our team.
